LDL plays a role in transporting cholesterol from the liver to the cells, but when it is present in excessive amounts, unused cholesterol may be deposited in the blood vessels.
Conversely, a sufficient level of HDL cholesterol, often described as “good cholesterol”, is considered a protective indicator. HDL acts as a cleaning system: it collects excess cholesterol from peripheral tissues and artery walls and carries it back to the liver, where it will be recycled or eliminated. A high HDL level is therefore associated with better regulation of the lipid profile and protection against cardiovascular disease.
Endogenous cholesterol production is largely regulated by a key enzyme: HMG-CoA reductase, which is mainly active in the liver. This enzyme controls hepatic cholesterol production; when its activity is too high, it may lead to endogenous overproduction and therefore an increase in blood cholesterol levels, particularly LDL cholesterol, thereby promoting the risk of cardiovascular disorders.
However, controlling HMG-CoA reductase—whether through nutritional or pharmaceutical means—may negatively affect the biosynthesis of another essential molecule: coenzyme Q10 (CoQ10). Indeed, the metabolic pathways of cholesterol and CoQ10 share common steps, which means that inhibiting the former may also reduce production of the latter.
